Primary Care Networks

The NHS Long Term Plan puts primary care at the heart of plans for Integrated Care Systems (ICS). Primary Care Networks (PCN) will become the delivery vehicles for many community health services, bringing together General Practices working at scale with other local care organisations.

General Practice is better placed to co-ordinate the changes to workforce, business and relationships that the evolving Primary Care Networks (PCNs) will require, however (PCNs) can only succeed with the support and engagement of Practice Teams.  Understanding integrated care systems and the strategy set out in the NHS Long Term Plan is challenging.  Newly formed PCNs are entering new territories and need guidance and help to face these challenges.
